What Is BHRT?

“Bioidentical” means the hormones used have the same chemical structure as the hormones the body makes, such as estrogen, progesterone and testosterone. It describes the molecule, not how a therapy compares with other options, and it doesn’t mean a treatment is free of risks or side effects.

Any hormone therapy is a prescription. It starts with a medical review and is adjusted over time. Results vary from person to person, and no result can be promised.

Ways Hormone Therapy Can Be Given

Your plan uses the method that fits your health review and routine. Options can include:

  • Pellets: small pellets placed under the skin during a short office visit.
  • Injections: given on a set schedule.
  • Creams: applied to the skin at home.
  • Capsules: taken by mouth.

Which methods are available differs for women and men: see the women’s and men’s pages for details. Pellets are placed by Dr. Berkey or our NP under lidocaine, and you can go back to normal activity afterward. They typically last about 3 to 4 months.

How a Hormone Therapy Plan Works

1. Consultation

We talk through how you’ve been feeling, your health history and your medications.

2. Labs

Labs are drawn before you start to set a baseline, again about 2 months later in case the dose needs adjusting, and then twice a year.

3. A Prescribed Plan

If hormone therapy is a fit, you get a written plan covering the hormone, the dose and the method.

4. Follow-Up

After the first few months, check-ins and plan reviews happen every 6 months, alongside your twice-yearly labs. Telehealth follow-ups and refills are available.

What to Expect With BHRT

Before

Your first step is a free consultation. Bring a list of your medications and supplements and any recent lab results.

During

Pellet visits are short office visits. Creams and capsules are used at home on the schedule in your plan, and injections follow a set schedule. Injections can be given in the office or at home, and your first one can be done or taught in the office.

After

Side effects depend on the hormone and the method, and we go over them with you before you start. Changes tend to be gradual and are different for everyone, so there’s no set timeline. Tell us about any changes you notice between visits.

Follow-Up and Adjustments

Hormone therapy is adjusted over time. After the first few months, check-ins and plan reviews happen every 6 months, alongside your twice-yearly labs. We’ll compare how you feel with your lab results and change the dose or method if needed. Results vary from person to person, and no result can be promised.

Is BHRT Right for You?

Hormone therapy isn’t right for everyone, and the reasons differ for women and men. Your consultation and health review decide whether it’s a fit. The women’s and men’s pages list who is not a good fit for each.

Not sure where to start? Book a free consultation and we’ll talk it through.

BHRT FAQs

How often are BHRT follow-ups?

Hormone therapy isn’t a set number of sessions. After the first few months, check-ins and plan reviews happen every 6 months, alongside your twice-yearly labs.

It depends on the method. Creams and capsules are used daily, injections follow a set schedule and pellets are replaced over time. Pellets are placed by Dr. Berkey or our NP under lidocaine, and you can go back to normal activity afterward. They typically last about 3 to 4 months.

Bioidentical hormones have the same chemical structure as the hormones the body makes. The term describes the molecule. It doesn’t mean a therapy is risk-free, and we’ll go over the options and their trade-offs at your consultation.
